Preparation
Check mems (especially detect illusion and light), reagents, light sources, healing items, etc. It is strongly recommended to bring at least two people, in case of any mishap in the dumbwaiter and beyond.
A hellish vista
Watch out for NPK. From Winterborn Mansion recall, go 3N, 2U, N, 3E, 2U, 8W, S. An alternate route (less movement on third floor) is from Winterborn Mansion recall, 3N, 2U, N, 3W, N, W, search hidden north, N, E, N, N, U, search hidden up, U, S, search hidden south, bash south, S, S, W, S.
In the south tapestry gallery on the third floor, a dumbwaiter may randomly stop there. It will show up as a CPK exit west. The dumbwaiter is restricted to level 120, so while you aren't completely safe, at least heroes/archons cannot pursue you there. All rooms beyond the dumbwaiter are CPK as well.
Push the 'UP' button and wait about 15 seconds to go up the tower. If everything goes smoothly, an exit will open up west leading you to the demon.
Sometimes, the dumbwaiter may get stuck. If this happens, the path to A massive iron rooster opens up. Unfortunately, there is no way to exit that area short of death.
If you're fortunate enough to have a first or second classer with detect illusion that'll help, have him go 3N, 2U, N, W, N from recall. Push the button until it 'pops outward' and have him go S, 3W (any rooms farther west are CPK; exercise caution). This room is NPK and if he has detect illusion active, there will be a door leading to a CPK, presumably restricted to level 120. Have your assistant go south and 'pull pulley' to unjam the dumbwaiter. That will allow it to freely move up and down again.
A demon composed of eyeballs encounter
While the dumbwaiter is restricted to level 120, the tower spire is restricted to level 60 and below.
The demon is not aggressive. If you have a spellcaster, apply curse/faerie fire/poison/plague/weaken to the demon before starting battle.
The demon is well-built due to the innumerable layers of eyeballs shielding her vulnerable parts. It will take some time to whittle her HP down. She has three attacks, so make sure the tank can handle it. The demon will attempt regular bite attacks, so set defense to dodge if the tank has it. While she does not cast any spells, the demon will break bones in both legs and arms on occasion. Broken bones will result in a high decrease in accuracy, HP/ST regen, even HP/ST itself and a slower walking speed if your legs are broken. If you have any broken bones, you risk getting hit for massive damage, if not outright killed instantly if she targets a limb she's already broken.
The demon will also randomly hit people in the form for a large amount of hp (for a first classer). It is advised to not even attempt this boss without at least 600HP. At the time of me recently doing this boss (2018) this emote was hitting me around 500-600 hp per use. Sometimes he wouldn't do it the entire fight, other times it would do it two rounds in a row. If you get hit by this and are below 600HP you should flee out and heal.
The Mark of Forgiveness will be awarded to the person who lands the final blow. That person will also be the one who receives the dirty equipment, as it loads in the killer's inventory rather than her corpse. Holding and using the dirty equipment will randomly reveal it to be one of the pieces below the dirty equipment ID.
