Found the ghoul in a house in
a Ghost Town in Old Sigil Underground north of room id #18210323.
Unsure if he always spawns there or if there are requirements to make him spawn.

its supports have completely given out, leaving rotten wood and patches of
decaying roof all over the floor. Even the walls have started to come down,
causing an opening to the outside. Moss and lichen has claimed a foothold
here, helping to consume the rotting wood, as well. The smell is less than
pleasant. Perhaps you ought to leave to the relatively fresh air of the
cavern to the south.
